Is the rotation permanent?

Yes — the rotation is baked into the saved PDF. Different from just viewing rotated, which doesn't save.

Are my PDFs uploaded?

No. pdf-lib does the rotation in your browser.

Can I rotate by other angles?

Only 90/180/270. PDF spec only supports those four.
